Every holiday season the Spinal Injury Foundation
purchases toys for the children of patients with disabling chronic spinal pain
who are out of work due to their injuries.
Simply fill out the form below before December 6, 2006.
Ten forms will be drawn at random from all eligible forms submitted. If
selected, your present will be ready to be sent by December 12, 2006.
Criteria for eligibility:

Parent suffers from disabling chronic spinal pain

Parent is out of work due to their disabling chronic
spinal pain

Child is younger than 18 years of age
